School Age Enrollment and Bus Form

*Note: WCADCC is not liable for the child until he/she arrives at the program or after the child has left the program to walk to/from the Bus stop.

Complete the information below to provide us with details about after school activities your child is participating in. Please complete a separate Transportation and School Activity form for each activity.

Your child's safety is our number one priority. Early Childhood Education Program will not release children form the program without the above information in writing.

may be picked up and dropped off at a bus stop other than directly out front of the West Chester Area Day Care Center (WCADCC).

WCADCC will make every effort to privide an escort to and from the bus stops whenever possible, but due to fluctuating bus schedules, we may not always be able to meet the bus. Your child may need to walk to WCADCC independently.

By signing below, I and my child agree and understand that we assume the risks inherent in the school bus route, and with full knowledge of the risks, we agree to release and hold harmless West Chester Area Day Care Center and their employees and representatives, from claims arising or related to my child's journey between the Center and the school bus. I also understand it is my responsibility to provide WCADCC the bus number, stop location, and pick-up/departure times. This information is sent directly to you from the school district. WCADCC is not involved in any aspect of school transportation and have no influence or say in the bus routes or stops.

Furthermore, I understand that in the event of a bus arriving after 5:30pm, I am solely responsible for meeting them at the bus stop as the Center will be closed.

Please ensure that your child has proper weather appropriate attire for outside (rain jacket, boots, etc...)

If your child is not attending care in the morning or afternoon, it is required that you please call and let us know. All families are required to sign your child in and out every day.
